We accept most major insurances. We verify eligibility and benefits upon your arrival at our clinics with your insurance card. Click here for a list of our accepted insurance. Before your visit, we recommend calling your insurance to verify your primary care benefits.
Our team contacts patients to provide notification of results and referrals. Lab results are typically available within 3 to 7 days, depending on the specific test ordered; however, laboratory results may take longer to be reported to us in some instances. If you have not heard from our office within 3 to 7 days, please check your patient portal, message us through your portal, or call our team directly. If you’ve not heard from our referral specialists within 5 days, please get in touch with our referral team directly at (813) 616-8031
We offer self-pay discounted pricing at the time of the visit. For patients without insurance, an evaluation starts at $150. If diagnostic testing, procedures, or other services are required, these charges will be in addition to the evaluation cost.
To reschedule or change your appointment, please call our team at (813) 616-8031 and state that you would like to cancel or reschedule.
We are open Monday through Friday from 8:00 AM to 5:00 PM. Our goal is to evaluate and treat most patients efficiently, allowing them to return home within 30 to 60 minutes.
Patients are encouraged to request their records by either calling us or visiting our office.
